Abstract: Background: Various medical therapeutic options have been examined to non-invasive treatment of pterygium and also improvement of its-related sequels. There are a few evidences on effectiveness of doxycycline in the recovery of pterygium lesions. We aimed to assess the therapeutic effectiveness of this drug on pterygium in a sample of patients in Iranian society.

Methods: This pilot study was performed on 24 patients with pterygium that randomly assigned to receive doxycycline with the different doses of 250, 500, and 1000µg. The changes in the size of lesion as well as its stage and grade were assessed one day, one week, and one month after intervention.

Results: The mean height and width of the lesion gradually decreased within one month following-up although it’s not clinical this changes were dependent to the dose of the drug. The stage and grade of the lesion remained unchanged within one month of intervention. No cases of hyperemia or congestion, chemosis or lachrymation were reported during the evaluation of the lesion.

Conclusion: Subconjunctival injection of doxycycline reduces the size of the pterygium lesion and this improvement effect is dependent on the drug dose, although this reduction in size is not clinically. (Statistically significant but not clinically significant)
