The Effect of upper lid blepharoplasty on tear film: subjective and Objective Evaluation with the OSDI Questionnaire and Keratograph 5M

Abstract: To evaluate the effect of upper lid blepharoplasty on the tear film subjectively by Ocular Surface Disease Index Questionnaire and objectively by the corneal topographer Keratograph 5M.
Methods: A prospective study was performed of 47 eyes of 47 patients with upper lid skin excess who underwent upper lid blepharoplasty. Conservative upper eyelid blepharoplasty was performed by an en bloc resection of anterior lamellar tissue included skin, and a strip of preseptal orbicularis oculi muscle. The subjects were evaluated for symptoms of dry eye with OSDI Questionnaire and the eyes were imaged using the noninvasive tear breakup time tools of the Keratograph 5M preoperatively and also 4 and 12 weeks after surgery.
Results: The mean age of the subjects was 43.2 ± 7.5 years, all of them were female. ANOVA and Friedman test were performed to compare objective and subjective data. There were significant changes of Ocular surface disease index questionnaire scores over time(P=0.0001). Regarding indexes of keratography 5M , NIKBUT, tear meniscus height , redness and meibography showed significant changes postoperatively.
Conclusion: Upper lid blepharoplasty can cause subjective and objective changes in tear film and dry eye symptoms even with conservative skin-muscle flap resection.
